Robin!! Been catching up on the Arrow reactions. Hope you're enjoying. For the flashbacks, Ivo initially aims the gun at Sara, which is why Oliver stepped in front of her, but I'd say it was more of a first instinct sort of thing. So in a way he did choose to save Sara, but obviously wanted to save Shado as well.
