Commission Form (Test and Feedback)

As mentioned, during this month I will introduce Commission so, I have created a form for it.
below are the two links, You have to use sign in with a valid email address for the Form with GDrive while the other you don't have to.

Just fill in randomly and Submit.
Also, I would like your feedback on how it can be improved further.
