Able Homecare LLC, a leading Service Coordination Agency in the homecare field, is seeking a dedicated and detail-oriented Care Plan Writer to join our team on a part-time basis. This position is remote, at least for now. You will be responsible for creating comprehensive and individualized Initial Service Plans (ISPs) that accurately reflect the needs and goals of our clients. Your work will be essential in ensuring our clients receive the appropriate care and services to improve their quality of life.
RESPONSIBILITIES:
Develop and write individualized Initial Service Plans (ISPs) for clients based on assessments, medical records, and input from professionals.
Ensure all ISPs are detailed, accurate, and compliant with regulatory standards and guidelines.
Collaborate with service coordinators and family members to gather necessary information and insights.
Maintain confidentiality and protect sensitive client information.
Meet deadlines and manage multiple ISP projects simultaneously.
REQUIREMENTS:
Excellent writing and communication skills with attention to detail.
Strong organizational skills and the ability to manage multiple tasks.
Proficiency in Microsoft Office Suite and other relevant software.
Ability to work independently and as part of a team.
PREFERRED QUALIFICATIONS (NOT REQUIRED):
Experience in healthcare, social services, or related fields.
Experience in writing care plans, ISPs, or similar documents.
Knowledge of Medicaid, NHTD Waiver Program, and related services.
COMPENSATION:
$200 per ISP written.
Writers can expect to complete at least one ISP per day, with the potential to complete up to two ISPs per day based on efficiency and workload, allowing for higher earnings.
ADDITIONAL INFORMATION:
This is a part-time position to start, with compensation based on the number of ISPs completed.
