Looking for someone to help me create an anonymous feedback form for my private consultations

My lack of technical expertise is showing up again. I've been doing a bunch of skype sessions recently and I want to offer people the chance to give me anonymous feedback on their session. As well as improving my skills, I'll be able to use any positive comments for promotional purposes.
Does anyone know how to set something like this up? I send someone a link and they write down feedback anonymously, maybe even fill out a small survey and I can view the results? If this is something that needs to be custom designed, I'm happy to pay a small fee for someone to set it up for me.
If anyone has any tips, please let me know. Thanks in advance brothers :)
