In the end, Tenma stays true to his character. He saves lives, he doesnt take them. If Tenma shot Johan OR chose not to operate on him he would be proving Johan's philosophy right. That all humans are only equal in Death. That a Monster has no way to redemption or can't ever be forgiven. That they should not be afforded the same basic right to life as anyone else. These are all things Johan has internalized and come to believe about himself, hence his choice to end his life and proof of ever having existed in the most twisted way possible. Its all he knows to do because of his upbringing and because he has no idea how to deal with the feeling of lacking a sense of self, an identity. He wants Tenma to be the only one left who can see the world how he sees it, barren and alone. The Landscape of the End I'm glad the writer chose this rather than the nihilistic route of having Tenma shed his values in order to enact capital punishment on a broken human being. Dont know if yall picked up on it, but the writer was poking at that idea of identity with the old couple who won the lottery. The man couldnt find his identification but is ultimately called back by his wife who reassures him that it's okay. Alluding to the fact that whether or not one has an "identity" doesnt matter because "people can become anything" just like Johan, Roberto, Nina and Grimmer. That the only thing which truly matters is the love from those near to us.
