another example of only 1 out per ball - lets say the ball hit the leg and the fielders appealed for Leg Before Wicket (LBW) and umpire gave it not out. The batsman, if possible, would still try and run and then the fielder throws at the stumps for a run out. Then the fielding team may use Decision Review System to check if the umpire's on field decision of not out for LBW was right. This will happen before checking for the run out on the big screen (as it was the order of play). Now if the LBW decision overturns and is actually out, the run out is disregarded. If the LBW decision is the same not out, this is when they will check for the run out.. hope this makes sense.

the one with the 2 outs - cricket laws state you can't have 2 outs. you can only have 1 out per ball. So going by this theory, the reason the bowler chucked the ball back at stump means he knew the batter wasn't out when he caught the ball (because it was a bump ball - the ball had hit the toe end of the bat and the ground almost instantaneously which is not always visible to the naked eye)
