Ripping out Y'Shaarj was a "superficial" wound, like ripping out a wart or growth on the skin - hurts a lot, lots of blood, but that's about it. And like Rossi said, the Titans treated that wound. But the Sword in Silithus, that's a deep stab wound, Azeroth is literally dying from it. That's gonna produce a different immuno-reaction. We also can't forget that there is likely some amount of Old God influence/essence in the Azerite we see - it could be that it's actually less blood, but pus.

I'm kind of with you. I suspect that Sylvannas Windrunner, Ranger General of Silvermoon died once and IS DEAD. Who Sylvannas is now is The Banshee Queen, a fundamentally different person (than the "fool" that she used to be). The Banshee Queen has only died twice and is, perhaps, ripening towards her third death.

One thing to consider with Sylvanas and the quote/whisper about “her third death†is that her death count AFTER being raised by Arthas is two. She died once as a mortal at the hands of Arthas and twice as an undead (Icecrown and Silverpine).
