Rift Reaver

Weapon (javelin or spear), very rare (requires attunement)

The Rift Reaver is a living, worm-like creature with the extraordinary ability to manipulate the fabric of space. Originating from the astral sea, it possesses the power to create spatial folds, allowing it to leap instantaneously from one location to another by forming rifts. Rift Reavers are often found near Outsiders that have breached the planar veil into the material world or around leyline nexuses where the boundary between the material plane and the astral sea is thin. Though generally harmless unless provoked or in large numbers, Rift Reavers feed on arcane energy and occasionally prey on small creatures like insects, birds, and rodents. Their unique affinity for arcane energy has caught the attention of spellcasters, particularly those who specialize in chronomancy and graviturgy. Many such mages seek out Rift Reavers as familiars, as the creatures naturally emit a specific frequency of arcane energy that aids in the casting of spells related to time and space.

While you are holding or wielding the Rift Reaver, it forms into a javelin. You gain a +2 bonus to attack and damage rolls made with this magic weapon.

Rift Strike. When you make a ranged weapon attack by throwing the Rift Reaver in javelin form, it propels itself through a tear in space. This attack ignores half cover and three-quarters cover. On a hit, the attack deals an extra 1d8 force damage. Additionally, once on each of your turns when you hit a creature with this attack, you can force the target to make a DC 16 Charisma saving throw. On a failed save, the target can't teleport or travel to a different plane of existence until the end of its next turn. After the attack, the Rift Reaver immediately returns to your hand.

Spatial Jump. Immediately after making a ranged attack with the Rift Reaver, you can use a bonus action to teleport to an unoccupied space within 5 feet of where the weapon struck. You can use this feature a number of times equal to your proficiency bonus, regaining all expended uses when you finish a long rest.

Symbiotic Familiar. If you know the find familiar spell, you can perform a special ritual over the course of 1 hour, which can be done during a short rest, to bind the Rift Reaver as your familiar instead of summoning a new creature. When you are not holding or wielding the Rift Reaver, it acts independently but always obeys your commands. In combat, it rolls its own initiative and acts on its own turn. The Rift Reaver can't attack, but it can take other actions as normal.

Oh, 'familiar can't attack' is the default for 'find familiar', but I still give it an attack in case you want to encounter it as a creature/enemy or if you have any feature that lets your familiar make their attack (like Pact of Chain Warlock's Invocation), or if your DM would allow it to still attack even as a familiar (in my experience some DM allow it either in combat or out of combat during roleplay moment)

Honestly, an awesome weapon-would love it on a dimensional traveler. A bit confused on why it has an attack as a familar when Symbiotic Familar says it can't attack, though.
