Itachi did actually know Kakashi had Sharingan, but was guessing Kakashi had reached a point in improving his own Sharingan to be able to tell a person's eyesight got worse over time 'because' of the drawbacks of using powerful Sharingan, which gives the clue that Kakashi was able to improve his Sharingan.
