Then we have multiple branching crews that work under the family. Think of it like a regular organization in an office job. In this office organization we have different departments that help run the different aspects of the company. You have the manager of the department and the workers who work under them and report to them. That's how the crews work in the mafia. You have captains (or capo's) who manage their crew, and the low level soldiers who make up the crew. The soldiers report to their captains, the captains report to the underboss or the boss himself. Regarding your confusion on who the group of guys Tony was eating seafood with in the strip club.. they are the other capos of the family. Tony and Junior are capos of their own crews, and as such Tony was having a discussion on what should be done about the Junior situation and who should succeed Jackie as The Boss. You were mistaken at the end. Tony has become the boss... unofficially. They gave Junior the title to resolve and de-escalate the situation, but in truth Tony is the boss and everyone reports to him. If you didn't notice, Tony got the idea from Dr Melfi. While discussing Tony's strained relationship with his mother and the issue with Junior, she gave Tony the idea of "giving them the illusion of being in control." That's why at the end, when Melfi asked if he still had doubts about therapy he said "no, I'm getting some good ideas from here."
