In a fantasy setting, such drugs are often replaced by magic (or magic items). I observe that Japanese hentai readers want to see not only death acme per se but also the very process that a woman is being defeated (or conquered) by sexual pleasure while she clearly knows that the organism process will kill her.

I can barely describe the invigorating feeling of seeing the girl experience one last sexual release via cumming or birthing, followed by the nosebleed and cold stare implying death. I wish it could end with a smile sometimes, but I can't complain. The drugs speeding up the process is also something I cannot complain about, as there aren't many other death-acme works that I've seen not involving the usage of drugs to speed up the orgasmic process. If you're knowledgeable about the Japanese hentai, you may know the theme 'death acme' (デスアクメ). If not, I believe this theme will satisfy you very much. One of the top hentai artists specializing in this theme is perhaps Naniwa-Michi sensei (https://www.pixiv.net/users/1045295). I also want to draw the death-acme theme someday, yet this theme is, personally, very difficult to realize since it requires a long 'background' story of why a lady embraces an extreme orgasmic ecstasy while reaching it entails her immediate death.
