Technically the position of Empress Regnant doesn't exist in the Li Empire (this fictional not China) as it's a patrilineal monarchy but the people addressed her as if she was, calling her Empress/"Jotei" (女帝) instead of Koutaigou/Empress Dowager (皇太后) or Mother of the Nation/Kokubou (国母).

Without giving spoilers, there are actually 2 empresses involved: - Empress Dowager Anshi, mother of the current Emperor - simply "empress": this is the mother of the previous emperor. (Netflex's translation gave a proper title to this empress though) Let's wait and see if they would clarify it a bit in the next episode, otherwise it would be really confusing.
