Just looked and it's still there for me. In the UK if that makes any difference. Episode 23 was blocked in the UK and had to be re-edited and uploaded and until it was, it wouldn't show up on their channel for me but was still on their playlist, which is how I found out it had been blocked.

The woman who asked Hohenheim to drink is Pinako Rockbell (Winry's grandma). In his dream, Pinako is like his subconscious, who keeps on encouraging him when he had doubts. While Pinako/his own consciousness acted like an angel on his shoulder, the Hohenheim with no glasses acted like his devil.
The little girl who asked Hohenheim for a dance was actually Trisha (his wife). Remembering Trisha had motivated Hohenheim to continue on with what he must do.

Yeah, the drinking and smoking girl was Winry's grandma when she was younger (she's so short now because this is an anime - so you have to use anime logic :)). The younger girl with the short hair talking to Hohenheim before the transition to Trisha in the field was just a younger version of Trisha. It's implied that Hohenheim met her at that age, and that it was the beliefs she expressed about humanity in that scene that caused Hohenheim to fall in love with her. Your other questions will be answered soon, but they're too spoilery to go into now :).
