Well, literal death is the side effect of the red pill, not metaphoricaly. The curse mark works in 2 stages/states. First state is the one we've seen before Sasuke using and these guys, with the black markings. The second state is the transformation this guy did. Also, for Choji's clan, they can transform calories into chakra, so eating boosts their fighting stamina. The big guy might do something sinilar also since he uses "eating" for absorbing/building chakra. An yes, their dads know eachother. We saw them in a restaurant togather at some point and they are the previous Ino-Shika-Cho team.
